Steps to Enroll
Log on to Benefits Self Service at HologicBenefits.com from May 15 to May 31.
- Return users, log in using your previously created username and password (This is NOT your network credentials).

- If you forgot your login credentials, click on the “Forgot…” link. Do not contact Hologic IT. If you still need further assistance, call the Benefits Service Center at the number to your right.

- New users, register with your social security number, date of birth, zip code, and password “hologic” (all lowercase) to create your unique username and password.
- There’s an App for that! The Benefits Service Center app called “MyChoice” is now available on your Company issued smartphone and tablet, or within your personal phone’s app store. However, you must first login to HologicBenefits.com and click on “Access the App” to scan your personalized QR code or obtain an Access Code.
Once you complete your enrollment, print a Benefit Summary for your records.
Important! Dependent Coverage Requirements
Be prepared to provide proof of your relationship to the dependent you are adding to your medical, dental, or vision plans, such as a birth certificate (for a child), or marriage certificate (for spouse/partner), or last year’s tax return (child and/or spouse/partner) in addition to having social security numbers available. Click here for a list and definition of eligible dependents. These documents are due by the end of Open Enrollment, May 31. Late submissions will NOT be accepted.
